Trace the answer
A generated answer can include an accurate sentence and an unsupported recommendation in the same paragraph. Review the material claims individually. For each one, identify the source, its freshness, contradictory information, and the effect the claim could cause.
Test exceptions
A representative evaluation set includes routine work, missing information, conflicting sources, unusual users, and deliberate misuse. Record which conditions produced escalation, denial, or a wrong answer.
A test result applies to the version, data, environment, and case set that produced it. A provider or prompt change can invalidate the inference.
Assign the next decision
An evidence gap has an owner and an exit: gather a document, run a test, narrow scope, request approval, or stop. This is how observation becomes a governable operating process rather than a collection of logs.
